Saint John’s Health Documentation Guidelines

 

                Below are the Saint John’s guidelines associated with the health records of our students.  To help us obtain the necessary documentation in order to best serve our student population, we have devised an Annual Student Health form.  The form is divided into “Part A” and “Part B” and both sections are referenced below.  An electronic version of the form is available at the bottom of this page.  Thank you to our parents for ensuring that we have up-to-date health information relative to your son(s).

 

Part A – Annual Student Health Form

                Each year, every student at Saint John’s must submit an updated Student Health Form.  By providing the required information, parents and guardians are equipping Saint John’s with crucial health information that will be utilized in the event of a medical emergency involving their son. This information can be submitted by parental completion of “Part A” of the Annual Student Health form.  All parents and guardians must complete and submit “Part A” of the Annual Student Health form at the beginning of each school year.

 

Part B – Annual Student Health Form:

                “Part B” of the Annual Student Health form must be completed by a physician and is done so at the time of a physical examination.  The following students must have “Part B” completed and on file at Saint John’s.

  • All new students (freshmen and transfer students)
  • Students who are participating in Athletics and who have not submitted a physical examination report to Saint John’s within the past 13 months

 

Physical Examinations:

                Upon enrolling at Saint John’s, all students must submit documentation associated with a Physical Examination.  This physical examination report will be sufficient throughout the student’s tenure at Saint John’s, with the exception of our student-athletes.   As noted above, all student-athletes must submit an updated physical examination report to Saint John’s every 13 months.
